Chapter Eternal Board Notification Form

The Alumni Board and the membershipo have established a Memorial Brick Fund to memorialize brothers who have passed on to the Chapter Eternal.

In order to make it easier to notify the Alumni Board of the passing of an initiated Brother on to the Chapter Eternal, the following form has been created.

Any Notifications between June 1 of each year through May 31 each year, the Alumni Board will collect notifications.

In May of each year, the Board will authorize the funds to order the Memorial Bricks to meet the date established by the Alumni House at Slippery Rock University so that bricks will be placed before Homecoming in October.
